Submit a new RCRV Project request

Nonprofit and social services agencies may submit a request for an RCRV project by filling out this form and returning it to RCRV by:

• Attend an RCRV meeting which meets at 10:00 every Thursday at the RCRV office, 927 N. Compton Drive, Hiawatha, Iowa.
• Mail to RCRV at 927 N. Compton Drive, Hiawatha, Iowa 52233
• FAX to 393-1250.

Requests may also be entered directly on the RCRV web site at www.collinsclubs.com/RCRV.  You must register for database access before data entry on the web is allowed.  Registration may be made from the web site.

Enter the requesting agency

Enter the name, phone number and e-mail address of a contact person that can answer questions about the project

Date submitted

Enter a full description of the project requirements.  Enclose supporting information such as drawings, photos, date required, etc if appropriate.
